Our work is possible only through our partnerships with health care providers, non-profit organisations, residential aged care providers, government, community and cultural organisations, philanthropic organisations and tertiary education.

In 2022/23 we:

Funded the culturally safe Bit-by-Bit Brief Pain Self-Management program.

Awarded grants to residential aged care facilities in the Southern NSW, South Western Sydney and ACT regions, to support the purchase of telehealth infrastructure to connect with health care providers.

Awarded virtual care grants to build capability in residential aged care in the use of telehealth and digital health tools (such as My Health Record, National Residential Medication Charts, Shared Care Planning Tools, and Secure Messaging).

Commissioned a system review and re-alignment project, highlighting ten key recommendations to improve Aboriginal end of life and palliative care services across the region.

Collaborated with the NSW Ministry of Health and general practice on the LUMOS program, to securely link de-identified data from general practices to other health data in NSW to shine a light on the patient journey.

Developed a care pathway for chronic obstructive pulmonary disease (COPD) patients through a collaborative commissioning project with Illawarra Shoalhaven Local Health District (ISLHD), and Southern NSW Local Health District (SNSWLHD).

Represented in the Illawarra Shoalhaven and Southern NSW Local Health District HEALTHPLAN (emergency management) plans.

We joined the Illawarra South Coast Regional Emergency Management Committee and the Eurobodalla Local Emergency Management Committee meetings as an observer, to link primary care with the emergency response of other regional healthcare and community organisations.

Nurse immunisers get a boost

Our region’s residential aged care facilities are now better prepared to provide vaccinations to older people in their care. More than 3230 new nurses received funding from COORDINARE to complete training to become authorised nurse immunisers.

Preparing for emergencies

We continue to partner with both Local Health Districts (LHDs) towards a one health system response to emergencies and disasters. Our regular meetings with Disaster Managers from both LHDs enable us to develop a shared understanding of our roles and responsibilities in disaster planning and recovery across the region.

Our collaboration has led to: 

  • training to support disaster preparedness in general practice  

  • coordinated response to the COVID 19 pandemic  

  • approaches that support vulnerable populations during disasters 

  • better recognition of the role of general practice in Health Service Emergency Management Plans and subplans.

Bit-by-bit brief pain management program

A collaborative codesigned project is improving access to culturally safe pain management information and skills for Aboriginal community members across our region.

The program is codesigned by local Aboriginal Health Workers and Integrated Team Care staff with support from the Pain Management Research Institute, the NSW Agency for Clinical Innovation and Illawarra Shoalhaven and Southern NSW Local Health Districts.

Locals join together to prevent suicide

Building on the experience of the Illawarra Shoalhaven Suicide Prevention Collaborative, this year we partnered with local organisations and community members to launch the Bega Valley Eurobodalla Suicide Prevention Collaborative.

Both Collaboratives are working in their communities to build a compassionate, skillful and sustainable approach to local suicide prevention.
