COORDINARE works with consumers and communities to identify local needs and engage in health service planning, design, delivery, and evaluation.
By working in partnership with consumers we ensure commissioned programs improve healthcare and health and wellbeing outcomes across our region. Community data, needs and preferences guide the work we do, to ensure services suit local people and local communities.

Supported people living with Chronic Obstructive Pulmonary Disease (COPD) to improve digital skills and better self-manage their condition through the Living Connected program.

Funded La Trobe University and Palliative Care NSW to deliver the Capacity HELP (Healthy End of Life Project). They worked with Shellharbour City Council to create four short films as a Dying to Know Day initiative.

Funded Multicultural Community Council Illawarra (MCCI) to develop Advance Care Planning videos in Macedonian, Portuguese and Vietnamese featuring local community members from various cultural backgrounds.

Weavers support through shared experiences

Weavers is a peer support program that connects people who are caring for a loved one with someone who has first-hand experience of being a carerCarers are matched with 1:1 flexible peer support from someone who has been a carer.

This year COORDINARE funded 3 organisations to deliver Weavers locally.
