COORDINARE works with consumers and communities to identify local needs and engage in health service planning, design, delivery, and evaluation.
By working in partnership with consumers we ensure commissioned programs improve healthcare and health and wellbeing outcomes across our region. Community data, needs and preferences guide the work we do, to ensure services suit local people and local communities.

Living Connected program
People living with Chronic Obstructive Pulmonary Disease (COPD) were supported to improve their digital skills and better self-manage their condition through the Living Connected program.

Weavers is a peer support program that connects people who are caring for a loved one with someone who has first-hand experience of being a carer. Carers are matched with 1:1 flexible peer support from someone who has been a carer.
